Robert Kaplan, M.D.

Doctor Robert Kaplan has been with America’s Disabled Physician Home Visits since 1991.  He received is Medical degree from Northwestern University Medical School in 1979 and completed his residencies at the University of Illinois Medical Center and the Veterans Administration Hospital.  Additionally, he has extensive research experience as both a Principle-Investigator and Sub-Investigator in various medical research studies.

 

George Dietz, M.D.

Doctor George Dietz received his medical degree from Stritch Medical School in 1956 and has extensive experience practicing medicine.  He has worked as a physician for numerous area hospitals and has held a number of teaching assignments during his career. He is a Charter Diplomat of the American Board of Family Practice and was a Charter Fellow of the American Academy of Family Physicians. He joined our staff in 2007.

 

Margaret Cozzi Olson, D.P.M.

Dr. Cozzi Olson graduated from the University of Illinois in 1977 with Bachelors degrees in Biology and Psychology and earned her Doctor of Pediatric Medicine from the Scholl College of Pediatric Medicine in 1981. She has been a practicing pediatrist for 30 years. For fourteen years she practiced in an Oak Park office, during which time she also serviced various nursing homes and halfway houses. For the past seventeen years she has operated a house call practice, primarily servicing the home bound erderly throughout the Chicagoland area. As of 1999 Dr. Cozzi Olson has also been practicing in the Oak Park office of Dr. Dean Stern. In addition, Dr. Cozzi Olson also services various retirement communities, including the Moorings of Arlington Heights, The Oakton Arms in Des Plaines, The Altenheim of Forest Park and Melrose Commons in Melrose Park.

 

Thomas Albert, M.D.,D.C.

Thomas Albert, M.D.,D.C., is proud to be a part of the Jefferson Park Medical Group team as a board certified internal medicine physician. Prior to becoming a medical doctor, he spent over 15 years practicing chiropractic medicine in the Chicago area. In addition to his past experience in musculoskeletal and pain medicine, he has extensive experience in the diseases that affect adults including obesity, high blood pressure, diabetes, heart disease, cancer and stroke. Dr. Albert’s background includes working both in the Loyola University and Ingalls Hospital emergency medical system as a paramedic. He has shared his passion for health and fitness as a consultant to schools, churches and community organizations throughout Chicago.

Additionally, Dr Albert has enjoyed teaching medical students, nurses and residents as an associate professor of medicine at Loyola University’s Stritch School of Medicine and West Suburban Medical Center’s Internal Medicine Residency Program.

Dr. Albert and his wife, who is a nurse, founded Abundant Health Ministry at New Life Covenant Church, Chicago. As both a medical physician and chiropractic physician, Dr. Albert combines the best of traditional and alternative methods to help patients achieve their maximum level of health. Dr Albert maintains membership in the American College of Physicians, the American College of Sports Medicine, and the Christian Medical and Dental Association.

 

Lisa Tutaj, PA-C

Lisa Tutaj is our newest Physician Assistant. She graduated from John H. Stronger Hospital - Malcolm X College Physician Assistant program, where she earned her Physician Assistant Certification in 2012. She comes to us with many years in various capacities in the medical field, including social worker. Lisa has a Master degree in Medical Science. She loves helping patients, and volunteers with several program citywide.

 

Hamdy Elhadary, MMS, PA-C

Hamdy Elhadary joined our group in 2012. Hamdy earned his degree at Malcolm X College Physician Assistant program, and he comes to us with 15 years experience in the medical field. He has held numerous positions at Northwest Memorial, St. Francis Hospital in Evanston, as well as North Shore, Evanston, Mt. Sinai and John H. Stronger Hospitals.

 

Richard Ansfield, President

Richard is the founder of America’s Disabled Physician Home Visits and has an extensive background in the healthcare industry.  He has been involved in the day-to-day operations of the organization since its inception and continues to work to grow the organization. Richard has also been involved in ancillary healthcare businesses for over twenty years.

 

Office Staff

Gosia Kocol-Markiewicz

Gosia, an associate of Total Healthcare and Clear Image Diagnostics prepares the routes and handles all of the paperwork for our medical staff. She is also the intake person for America’s Disabled.

 

Melissa Rybaltowski

Melissa, an associate of Jefferson Park Medical Clinic, assists America’s Disabled Physician Home Visits in organizing referrals for our patients for any ancillary treatment that the doctor recommends after their home visit. She also schedules visits for our patients. 

 

Debbie Fritts

Debbie, an associate of THC assists Gosia and Melissa in coordinating home visits and handles bookkeeping duties.
